Objective To evaluate the modes of death and treatment offered in the last 24 h of life to patients dying in 10 Brazilian intensive care units (ICUs) over a period of 2 years. Design and setting Cross-sectional, multicentre, retrospective study based on medical chart review. The medical records of all patients that died in seven paediatric and three adult ICUs belonging to university and tertiary hospitals over a period of 2 years were included. Deaths in the first 24 h of (...) admission to the ICU and brain death were excluded. Intervention Two intensive care fellows of each ICU were trained in fulfilling a standard protocol (κ=0.9) to record demographic data and all medical management provided in the last 48 h of life. The Student t test, Mann–Whitney U test, χ2 test and RR were used for data comparison. Measurements and main results 1053 medical charts were included (59.4% adult patients). Life support limitation was more frequent in the adult group (86% vs 43.5%; p<0.001). A ‘do not resuscitate’ order was the most common life support limitation in both groups (75% and 66%), whereas withholding/withdrawing were more frequent in the paediatric group (33.9% vs 24.9%; p=0.02). The life support limitation was rarely reported in the medical chart in both groups (52.6% and 33.7%) with scarce family involvement in the decision making process (23.0% vs 8.7%; p<0.001). Conclusion Life support limitation decision making in Brazilian ICUs is predominantly centred on the medical perspective with scarce participation of the family, and consequently several non-coherent medical interventions are observed in patients with life support limitation. Psychopathy is a disorder characterized by severe and frequent moral violations in multiple domains of life. Numerous studies have shown psychopathy-related limbic brain abnormalities during moral processing; however, these studies only examined negatively valenced moral stimuli. Here, we aimed to replicate prior psychopathy research on negative moral judgments and to extend this work by examining psychopathy-related abnormalities in the processing of controversial moral stimuli and positive moral processing. Incarcerated adult males (N = 245) completed a functional magnetic resonance imaging protocol (...) on a mobile imaging system stationed at the prison. Psychopathy was assessed using the Hare Psychopathy Checklist–Revised (PCL-R). Participants were then shown words describing three types of moral stimuli: wrong (e.g., stealing), not wrong (e.g., charity), and controversial (e.g., euthanasia). Participants rated each stimulus as either wrong or not wrong. PCL-R total scores were correlated with not wrong behavioral responses to wrong moral stimuli, and were inversely related to hemodynamic activity in the anterior cingulate cortex in the contrast of wrong > not wrong. In the controversial > noncontroversial comparison, psychopathy was inversely associated with activity in the temporal parietal junction and dorsolateral prefrontal cortex. These results indicate that psychopathy-related abnormalities are observed during the processing of complex, negative, and positive moral stimuli. (shrink)

Newman, en la que se discute la teoría de los conjuntos , propuesta por el matemático Georg Cantor , y mediante la cual se crea la aritmética transifinita y se establece un sistema epistémico para representar los diversos niveles del infinito. Así, Cantor le asigna a estas infinitudes la primera letra del alfabeto hebreo, el Aleph, seguido de un determinado número, dependiendo del nivel (...) de infinitud . Borges, de esta manera, teje varias de sus narraciones en las que se trata el tema del infinito y del absoluto; un ejemplo de ello es la colección de relatos bajo el título El Aleph, que abre con el cuento "El inmortal" y cierra con el quele da el título a la colección. Este ensayo tiene el propósito de estudiar "El inmortal" bajo la óptica cantoriana, para hablar de un absoluto en particular: el yo, y sugerir que no es posible establecer un vocabulario final, o una definición definitiva sobre el tema en cuestión. Esta imposibilidad, propone Borges, está dada en parte por la finitud lingüística, mientras que por otro lado la falibilidad de la memoria juega también un papel crucialen todo intento de definición. Sin embargo, como buen ironista, Borges, a través de "El inmortal", es capaz de proveer una redescripción del tema en cuestión mediante un lenguaje transfinito, sin pretender establecer un vocabulario final sobre el tema sino, al contrario, tratando de resolver ciertas paradojas a la vez que revela otras, promoviendo de esta manera el permanente diálogo entre las distintas disciplinas. Aunque este ensayose enfoca en el análisis de "El inmortal", a fin de desarrollar el tema propuesto, también estudia otros relatos contenidos en El Aleph, y utiliza un acercamiento teórico que se enmarca en la filosofía de la lengua. Jorge Luis Borges often consulted Mathematics and Imagination, by E. Kasner and J. Newman, where set theory is addressed . This theory was proposed by Georg Cantor and by it transfinite arithmetic is established and an epistemic system is created to represent different levels of the infinite. This way Cantor labels the different levels of the infinite by assigning to each the first letter of the Hebrew alphabet, the Aleph, followed by a number, depending on level of the infinite he is referring to . Following these ideas, Borges weaves several narratives in which the infinite and the absolute are discussed. An example of such narratives is the collection of stories compiled under the title The Aleph, which opens with "The Immortal" and closes with the story that gives the collection its title. The objective of this paper is to study "The Immortal" under the Cantorian lens to discuss one particular absolute, the self, and to suggest that it is impossible to establish a final vocabulary, or a definite definition, about this topic. This impossibility, Borges proposes, is in part due to the apparent finitude of language while at the same time the fallible attributes of human memory also is crucial when it comes to defining anything. However, as the ironist Borges is, he is capable of providing through "The Immortal" a re-description of these issues by means of a transfinite language that resolves some paradoxes while at the same time reveals others. By this way of writing, I propose, Borges fosters the continuation of the dialogue among the different disciplines. Though I will center my analysis on "The Immortal", to develop these ideas I also revisit other stories contained in The Aleph departing from a theoretical approach rooted in the philosophy of language. A unifying theory of general anesthetic-induced unconsciousness must explain the common mechanism through which various anesthetic agents produce unconsciousness. Functional-brain-imaging data obtained from 11 volunteers during general anesthesia showed specific suppression of regional thalamic and midbrain reticular formation activity across two different commonly used volatile agents. These findings are discussed in relation to findings from sleep neurophysiology and the implications of this work for consciousness research. It is hypothesized that the essential common neurophysiologic mechanism underlying anesthetic-induced unconsciousness is, as with (...) sleep-induced unconsciousness, a hyperpolarization block of thalamocortical neurons. A model of anesthetic-induced unconsciousness is introduced to explain how the plethora of effects anesthetics have on cellular functioning ultimately all converge on a single neuroanatomic/neurophysiologic system, thus providing for a unitary physiologic theory of narcosis related to consciousness. He examines ways in which the Ancient Greeks dealt with questions about how and why things happen as and when they do, about the basic constitution and structure of things, about function and purpose, laws of nature, chance, coincidence, and responsibility.

This article presents the new German law on advance directives from 1 September 2009. The history of the parliamentary process of this law is described, the present regulations are explained, their relevance for medical practice discussed and shortcomings are identified. Finally, the new law is compared with other regulations in the international context. Previously established legal practice in Germany has now become largely confirmed by the new law: An advanced directive must be respected in any decision concerning medical treatment, regardless (...) of the stage of the illness. It can be informally revoked at any time, even with limited decision-making capacity. Nobody may be obliged to issue a directive in any way. Advance directives do not need notarisation or routine updating after certain time intervals. Provided that the patient, who is no longer mentally competent, has issued a lasting power of attorney, or provided that the patient has been appointed a healthcare proxy by the courts, this authorised surrogate must assert the patient's will. The role of the guardianship court is clarified: it only needs to be involved in cases of disagreement as to the patient's will. The new German law thus combines more legal certainty with a liberal emphasis on patient autonomy and flexible, adaptable regulations. (shrink)

The four short works in Untimely Meditations were published by Nietzsche between 1873 and 1876.They deal with such broad topics as the relationship between popular and genuine culture, strategies for cultural reform, the task of philosophy, the nature of education, and the relationship between art, science and life. They also include Nietzsche's earliest statement of his own understanding of human selfhood as a process of endlessly 'becoming who one is'. As Daniel Breazeale shows in his introduction to this new edition (...) of R. J. Hollingdale's translation of the essays, these four early texts are key documents for understanding the development of Nietzsche's thought and clearly anticipate many of the themes of his later writings. Nietzsche himself always cherished his Untimely Meditations and believed that they provide valuable evidence of his 'becoming and self-overcoming' and constitute a 'public pledge' concerning his own distinctive task as a philosopher. Sonja Plischke’s Die Seleukiden und Iran is an attempt to study Seleucid empire building in Iran. In view of the fact that indigenous written sources from Iran are practically absent, Plischke decided to include Babylonia in this work, since the written cuneiform evidence from that region is fairly abundant. She detects some general tendencies—such as religious policy and the general wish to connect the different parts of the empire—supported by a personal conception of Seleucid kingship, beside more specific treatment of (...) different regions. In my view, the Seleucids probably did not pay sufficient attention to the East, so that their control weakened. I take the opportunity to discuss the Babylonian sources, correct errors in Plischke’s use of them, and adduce some new publications. (shrink)

Ethics edifies to the extent it takes seriously the munus propheticum Jesu Christi. Though many assume ethical action indicates behaviour realising Jesus Christ, this is problematic because it implies he is otherwise mute and absent. Paul Lehmann offers a refreshing alternative when he argues that the principal concern of ethics is alignment with all that God in Christ is doing now to make and keep human life human. Lehmann thus recasts the question ‘What am I to do?’ by taking seriously (...) the presence and activity of the One who summons human beings to participate in his work. In a manner consistent with the christological foundations of Dietrich Bonhoeffer’s Ethics , Lehmann’s proposals evoke an ethic shaped by and subject to the self-disclosure and radiance of the triune God. Certains se souviennent d'un auteur de la fin du siècle dernier qui s'était rendu célèbre par ses extravagances vestimentaires : son pourpoint Renaissance et sa barbe à l'assyrienne dans le Paris de la Belle Epoque défrayait la chronique. Les journalistes ont aussi consacré une large place à ses démêlés avec d'autres occultistes. Ses salons (...) de la Rose-Croix, pour lesquels Satie composa des sonneries, eurent une grande renommée et sont importants pour qui s'intéresse à l'histoire de l'art. Péladan, avec un sens certain de la publicité, s'était proclamé S.A.R., titre assyrien qui en faisait un descendant mythique des Mages chaldéens. C'est ce titre qui a assuré sa notoriété jusqu'à nos jours. D'androgynes pervers en femmes violées, mêlant la magie aux thèmes décadents, ses romans présentent un tableau étonnant de la pensée fin de siècle. Ce texte veut démontrer que Joséphin Péladan, qui eut des admirateurs comme Jarry ou Paul Valéry, n'était pas qu'un dandy excentrique, un farfelu 1900 teinté d'ésotérisme mais un authentique écrivain. Il existe un récit de Borgès qui raconte l'histoire d'un peintre qui voulut faire un tableau de l'univers. Au bout de nombreuses années de travail, il avait peint un mur entier de personnages pour s'apercevoir qu'en fait il n'avait peint que son propre visage. Ce fut précisément la destinée de Péladan : l'auteur ne fit plus qu'un avec son personnage principal du Mage. Jamais son œuvre n'aura présenté une telle fusion entre imaginaire et réalité. (shrink)

Rather than viewing theseissues as problems, they should be viewed asopportunities for debate, learning aboutothers' views, and resolution. The UnitedNations has an important role to play in howdecisions are made in the global ethical debatein food and agriculture. The ethical role ofFAO is to promote global food security,balanced conservation, management andutilization of natural resources, andsustainable rural development. FAO should fullyand publicly assume its ethicalresponsibilities, gathering and sharinginformation on ethics in its areas of mandate,acting as an interactive forum, and providingexpert guidance on policy options and choicesbased on practical ethical analysis.
